Healthform Information
You will receive a Health Form at the end of your child's Physical. This form includes all immunizations and current measurements. The Health Form can be used for school, sports, and camp. We suggest that you make several copies of your child's Health Form to use through out the year. There will be a charge for additional copies of this health form
